Applications for the program are taken year round. Interested parties
may obtain information by calling (602) 254-5081. To
apply for services, visit our site located at 39 East Jackson Street,
Phoenix, Arizona 85004.
Acceptance into each course requires full participation in all aspects of
the training including the completion of study materials.
Notification of Acceptance
You will receive notification regarding assessment and considered
program acceptance. Sending in an application or request for
information does not guarantee acceptance and unless you have
received notification it should not be assumed that you are accepted.
In order to be considered for the program, the applicant must meet the
seven eligibility requirements listed. The applicant must document the
seven listed qualifications.
- Proof of
Citizenship:
A
copy of a birth certificate or other official document showing date of
birth such as a passport.
- Proof of Residency
A
copy of a utility bill, driver's license, AFDC award Letter, etc.
- Family size
A
copy of social security card (one for each person living with you), DES
award letter, etc.
- Photo ID Card
Driver's
license, Food Stamp ID, State ID, VA Card, etc.
- Proof of Household
Income (for last 6 Months)
Pay
stubs, DES Award Letter, Worker's Compensation Documentation, Social
Security Award Letter, Child Support/Alimony, etc.
- Selective Service
(For Males Born After Dec. 31, 1959)

- Names, Addresses
and Phone Numbers of 3 People Not Living at Your Address Who Will
Always Know How to Contact You.
When all documentation is available, the applicant may make an
appointment by calling Arizona OIC. All applications will be filled out
on-site. The applicant will complete the application form indicating the
program for which he/she wishes to be interviewed.
